Test/Calibration
Certificate issued from the laboratory contains
the following information as under :
1. Name and address of the firm/customer
on behalf of whom test fee is received.
2. Reference number of the forwarding letter/test
request with date.
3. Date of receipt of sample and issue of
the Test Certificate.
4. Description of sample including details
of the seal, if any, as found on the sample.
5. Specification number and year with Amendment/Amendments,
if any, as per which test/calibration is
undertaken.
6. Any deviation from the test/calibration
method, where relevant.
7. Test-wise results in sequence/calibration
results.
8. Remarks/comments regarding conformity
or otherwise of the sample based on the
test/calibration results obtained with reference
to the requirements of the relevant Standard
Specification.
9. Signature of the Scientist (Scientist
SB or Scientist SC as the case may be) of
the concerned Laboratory who accepts complete
responsibility regarding authenticity of
the contents in the Test Certificate.
10. Emboss with signature of the Divisional
Head.
Note :
1. No remarks /comments are offered when
the sample is not tested/calibrated as per
any Specification.
2. The result of test/calibration furnished
in the Test Certificate relates only to
the sample tested/calibrated.
3. Reproduction of Test/Calibration Certificate
is not permitted except in full without
the written permission from the Director/Incharge
of the concerned region.
4. The Test/Calibration Certificates are
issued to the clients by registered post.
Amendment
to or revision of issued Test/Calibration
Certificate :
To maintain uniformity in practice the situation
for issuance of amendment to or revision
of Certificate in case of an eventuality
is identified and, where necessary, the
same is issued against the Certificate already
issued by NTH so that the clients always
get the correct observations/results. The
procedure being followed is as under :
1. Such type of case, on receipt from any
client, is first referred to the Director/Scientist-in-Charge
who in turn sends the case to the Divisional
Head of the concerned Laboratory for investigation.
2. On investigating the case including re-testing
of sample/samples where necessary, if the
Divisional Head finds that there is no need
to issue amendment to or revision of already
issued Certificate he prepares a draft reply
indicating therein the findings of the investigations
made by him and put up to the Regional Head
for his examination and approval. On obtaining
approval from the Regional Head the same
is issued to the client by registered post.
3. If after investigation including re-testing/re-calibration
of the sample, where necessary, the Divisional
Head finds that changes are required to
be made in the already issued Certificate,
he prepare a draft revised Certificate under
the same number of the already issued Certificate
but with the added word 'Revised' and put
to the Regional Head for the his examination
and approval.
4. The Divisional Head after obtaining approval
from the Regional Head withdraws the original
Certificate from the client issued to them
earlier and instead issues the revised Certificate.
